Director of Clinical Operations

Job Overview

Responsible for orientation and supervision of clinical practices and objective in the field. Recommends clinical in service and educational programs, monitors program expenditures, patient quality of care standards and utilization from the field perspective. May interview and make recommendations for hiring, and promotion. Operationally responsible for quality performance utilization review and coordination of patient services in the field.

Requirements

Licensed as a registered professional nurse in the state of Michigan. Bachelor’s degree in nursing, biology or other health related field and a minimum of 2 years clinical experience as a Home Care Nurse required. Experience in quality assurance, ICD-10 coding and OASIS certification preferred.

RN Care Manager Kalamazoo

(Full-time)

Job Overview

The right candidate will preferably have homecare experience, be self-motivated, have the ability to work with minimal supervision, can think on their feet and have a positive attitude. The nurse is responsible for providing individualized comprehensive nursing care to clients within the UNS service area (Kalamazoo/Portage, Battle Creek and surrounding areas). The role of the RN Care Manager is to admit patients, set frequency visits, communicate with physician offices and obtain orders, work with therapy disciplines to coordinate care, perform routine visits, resumption of care and follow the patient through to discharge.
Applicants must be a Registered Nurse licensed to practice in the State of Michigan. Home Health experience preferred. Experience with electronic medical record systems a plus.
Must be available for on-call evening and weekend rotating coverage.

Physical Therapist

Job Overview

The Physical Therapist provides treatment to individuals to develop, maintain and restore maximum movement and function throughout life. This includes providing treatment in circumstances where movement and function are threatened by aging, injury, disease or environmental factors.

Requirements

Graduate of a physical therapy school approved by the American Physical Therapy Association (APTA) or the Council of Medical Education of the A.M.A. in the collaboration with APTA or licensed by the State of Michigan. Home Health experience preferred. Experience with electronic medical record systems a plus.
